Items Needed for Enrollment 

During enrollment, the following must be provided:

Child

•  Birth Certificate

• Physical Exam -  (Must be dated within the last 6 months)

• Immunization Record

Parent/Guardian

• Proof of Valid Photo Identification: Current Driver's License or State ID (accepted only)
